All the information we need to perform your service can be downloaded here. Currently, we are unable to process payments for pre-arranging a cremation. However, you are encouraged to download and prepare these documents in advance.

There are three forms you'll need to fill out:

Cremation Authorization- A form the next of kin prepares and signs authorizing Universal to perform a cremation

Vital Statistics- This is required to obtain a death certificate and a cremation permit

Release of Remains- Allows Universal to transport your loved one into our care
